Palliative care is specialized medical support for individuals living with serious or chronic illness — at any stage of their condition. Our team focuses on relieving pain, managing symptoms, and improving quality of life while you continue treatments or explore other care options. We’re here to support not just your physical needs, but also your emotional and spiritual well-being, so you and your family feel cared for every step of the way.
Hospice care is specialized support for patients and families facing a life-limiting illness, when the focus shifts from cure to comfort. Our compassionate team is here to ensure you or your loved one can live with dignity, peace, and comfort — wherever you call home. We focus on managing symptoms, easing pain, and providing emotional, spiritual, and practical support throughout this important stage of life.
Southcoast Health’s Supportive Care services include an expert team of physicians, nurse practitioners, certified hospice nurses, social workers, chaplains, bereavement counselors, certified nursing assistants and specially trained volunteers, providing compassionate care tailored to your specific needs.
Whether you need help with symptom management, supportive counseling, or hospice care, Southcoast Health is here to help. Our Supportive Care team offers both hospice and palliative care for individuals facing serious chronic or life-limiting illnesses, including but not limited to:
