At Southcoast Health at Home, formerly known as Southcoast Health Visiting Nurses Association (VNA), we treat your family like our own. For over 100 years, we have been proudly serving patients across Massachusetts and Rhode Island with compassionate, high-quality home health care. We offer a full range of services to individuals of all ages recovering from illness or surgery, and those living with disabilities — all delivered with the personalized attention you deserve in the comfort of your home.

Home Care

Our home care services bring skilled, compassionate care right to your door. We believe nothing is more important than your loved one’s comfort and safety at home. Working closely with your providers, we create a personalized plan to help you live safely and comfortably at home. 

Specialized Care

From advanced heart and cardiac care to joint replacement surgery aftercare, our interdisciplinary teams are highly skilled and expertly trained to provide advanced, specialized treatment patients require. 

Supportive Care

Our Hospice and Palliative care experts provide specialized support for patients with serious or life-limiting illnesses, whether you’re undergoing treatment or focusing on quality of life. We’re here for you at every stage of your journey.

Home care is professional medical and support services delivered in a person’s home. It typically includes skilled nursing, physical, speech, occupational therapy, medical social work, and home health aide assistance. We partner with local hospitals, physicians, and community agencies to provide the highest quality of care possible. Home care services typically begin when we receive a referral from your health care provider. Once the referral is received, our team will coordinate with you to arrange the appropriate level of care based on your needs.

Home care is often covered by Medicare, Medicaid, and many private insurance plans. Our Customer Care Center is here to guide you and your family through the coverage process get access to the appropriate level of care.

Remote Patient Monitoring (RPM), may be offered based on your specific health needs and your care plan. If appropriate, our clinical team will provide you with the necessary equipment and instructions to support your care at home.

Supportive care offers guidance, resources, and emotional support to patients and families facing serious health conditions, helping them manage symptoms, make informed decisions, and improve quality of life.

Many insurance plans, including Medicare and Medicaid, may cover all or part of supportive care services. Our team can help you understand your coverage and verify your benefits to ensure you get access to the necessary/required level of care.

If you or a loved one has an advanced chronic illness and are at risk of hospitalization, talk to your doctor about supportive care. It can help manage symptoms, reduce hospital visits, and improve your overall quality of life.
